Role Description
The MDS Coordinator is a full-time, on-site position located in Morrison, IL. The role involves managing the coordination and completion of the Minimum Data Set (MDS) assessments and care plans to ensure compliance with regulations and optimal resident care. Responsibilities include collaborating with the interdisciplinary team, assessing resident needs, developing individualized care plans, and ensuring accurate documentation.
